To qualify for NorthSTAR you MUST:

- If you do carry private insurance that does not provide mental health and/or substance use disorder treatment a letter from your insurance carrier stating such will be required.
- Proof of residency in one of the seven counties NorthSTAR serves (Collin, Dallas, Ellis, Hunt, Kaufman, Navarro, and Rockwall Counties)
- You MUST apply in person to become enrolled in NorthSTAR at one of several NorthSTAR Provider locations across the seven counties. To find the location nearest you please refer to the Provider Directory or contact ValueOptions at 1-888-800-6799
